Uganda Tests Medical Drones To Cut Delays in Island Healthcare As Kalangala Trials Advance

Health officials are testing drone flights between Butabika, Koome and Kalangala to improve medical supply deliveries and laboratory sample transportation.

Uganda’s Ministry of Health is testing drone technology to deliver essential medical supplies and transport laboratory samples from hard-to-reach island communities, in a move aimed at reducing diagnostic delays and improving access to healthcare.

The latest trial involved a medical drone flight from Butabika in Kampala to the Koome Islands, onward to Kalangala Town Council and back to Butabika, with the aircraft carrying medical supplies to island health facilities and returning with samples for laboratory testing.

The initiative was launched by the Minister of State for Health in charge of Primary Health Care, Dr Charles Ayume, as the government explores ways of using technology to overcome longstanding transport challenges affecting communities scattered across Lake Victoria.

In an update, Ayume said the flights would generate evidence to guide decisions on incorporating drone transportation into Uganda’s existing Integrated National Sample Transport Network.

“Evidence from these flights will inform decisions on how drone transport can be integrated into the existing Integrated National Sample Transport Network, strengthening timely and reliable sample transportation from hard-to-reach communities,” he said.

The trials are intended to establish whether drones can provide a dependable alternative to conventional transport systems that rely on boats and road vehicles to move medical commodities and patient samples between island health facilities and laboratories on the mainland.

Initial tests along the Butabika–Koome–Kalangala route showed that the drone could complete the journey within the expected timeframe, with the Ministry of Health assessing its performance ahead of a planned operational rollout.

Island communities face days of waiting for laboratory results

The drone programme addresses a major logistical challenge in Kalangala District, where communities are spread across numerous islands and many health facilities depend on water transport to connect with referral services.

At present, patient samples collected from island health centres are transported by boat to collection points before being transferred by road to laboratories in Kampala. The process can take several days, delaying diagnosis and treatment.

Frank Kisakye, the in-charge of Kalangala Health Centre IV, told Daily Monitor that the facility serves as a collection point for samples from 16 health facilities across the district.

He explained that samples collected on Tuesday could reach laboratories at Butabika on Thursday, particularly affecting investigations requiring specialised testing, including HIV and hepatitis tests.

The proposed drone service could reduce transportation time to approximately one hour on the tested route, potentially enabling samples to reach laboratories sooner and allowing patients to receive results and treatment decisions more quickly.

The technology could also reduce the logistical risks associated with transporting medical materials across Lake Victoria, where patients and health workers depend on boats to reach facilities on distant islands.

Four health centres targeted in initial rollout

The initial deployment is expected to cover Jaana, Bufumira, Kyamuswa and Bubeke health centres, with the Ministry of Health considering an eventual expansion to additional islands.

The project team has been mapping routes and collecting geographical coordinates for health facilities and potential delivery points to support the planned operations. Drone Plus is reported to be managing the project.

The flights are also intended to establish how drone transportation can complement the country’s existing laboratory sample collection and referral arrangements rather than operate as a separate system.

Integrating the technology into the national network could help health authorities coordinate the movement of samples from remote facilities to laboratories while improving the reliability of medical supply deliveries.

Uganda builds on earlier medical drone experience

Uganda has previously explored medical drone deliveries to address healthcare access challenges in island communities.

A medical drone initiative documented by the Academy for Health Innovation Uganda has focused on delivering antiretroviral therapy and transporting laboratory samples in Kalangala, where conventional boat transport can be costly, unreliable and vulnerable to bad weather.

Research presented at the Global Health Supply Chain Summit documented 443 flights between September 2021 and February 2024 under an earlier project serving island communities. The initiative demonstrated the potential of drone deliveries to support access to medicines, although broader implementation requires attention to cost, reliability and operational challenges.

Experiences from other African countries also offer lessons. Rwanda has used drones to transport medical products, while Kenya and Malawi have explored drone-based delivery and sample transportation initiatives. These experiences demonstrate possible applications but do not, by themselves, establish the likely results or cost-effectiveness of Uganda’s current programme.

Operational questions remain

Although the latest test flights mark progress in exploring alternative medical logistics, the long-term impact will depend on whether the service can operate reliably and be integrated effectively into existing healthcare systems.

Key considerations include the cost of regular flights, weather conditions over the lake, the types and quantities of medical supplies and samples that can be transported, and the arrangements for handling specimens and processing laboratory results once they arrive.

The Ministry’s stated approach is to use evidence generated by the flights to guide decisions on future integration into the Integrated National Sample Transport Network.

If the system proves reliable and affordable, drone transportation could help reduce delays affecting island health facilities and improve access to diagnostic services for communities that are difficult to reach by road.

For residents of Kalangala and the Koome Islands, the central test will be whether the technology translates into more dependable medical supplies, faster laboratory results and timely treatment—not simply successful flights.
